LESSON POLICIES, PROCEDURES AND RATES

TUITION PAYMENT SCHEDULE:
Tuition notices are handed out to the student on the last lesson of every month. If the student is not in attendance, the notice will be mailed. Tuition is due on the first lesson of every month WITH NO EXCEPTIONS. If the student is not in attendance at the first lesson of the month, payment must still be received at that time either in person, or by mail. If payment is not received WITHIN 5 DAYS of the scheduled lesson time the time can be forfeited and given to another student.
PAYMENT OPTIONS:
Tuition payments can be made either by cash or check. If paying by check, please make payable to Kim Bernat.
SCHEDULE:
Lessons in Oak Park are scheduled on a weekly basis every Tuesday, Wednesday and Thursday at the same time every week. It is the responsibility of the student to be at each week's lesson at the designated time, thus when initially signing up for lessons, please make sure you are choosing a time that will be easy to commit to without conflict. It is imperative for the musical growth of the student that there is a consistency in attending lessons every week with minimal interruptions.
CANCELLATIONS:
Notice of cancellation must be given 24 HOURS OR MORE before the scheduled lesson time. If less than 24 hours is given, REGARDLESS OF REASON, the lesson IS NOT CREDITED. If notice is given with 24 hours or more, you may make up the lesson at a mutually agreed upon time.
TEACHER CANCELLATIONS:
If I cancel a lesson for any reason, then the lesson will either be made up or credited forward.
MAKEUP LESSONS:
Make up lessons are given when there has been enough notice of cancellation (24 hours or more). The lesson will be scheduled at a mutually agreed upon time**.
** IF A MAKEUP LESSON IS CANCELLED BY THE STUDENT FOR ANY REASON, IT WILL NOT BE RESCHEDULED.
NO-CALL/NO-SHOW:
Failure to attend 2 lessons in a row without notification will be considered a withdrawal from lessons and any remaining tuition is forfeited.
MATERIALS:
Students are responsible for bringing ALL pertinent materials to class, every week without exception.
PRACTICING:
It is important that the student show continuous growth in all areas of his or her musical endeavors. While I do not have a strict practice policy it is necessary to realize that the amount of time spent practicing during the week away from studio is indicative of how much progress is made in studio.
AGE RANGE:
Students of voice are accepted from age 9 and up. Students who wish to study voice and are between the ages of 7 and 9 will be given 2 evaluation lessons (Regular lesson rates apply) to determine if they are ready to study. In some cases, piano lessons will be recommended for up to one year and then another evaluation will be done at that time.
Students of piano from beginning to intermediate are accepted from age 7 through adult.
Parents are invited to sit in on the first two lessons only and then only on an as needed basis.
PLEASE READ THIS DOCUMENT CAREFULLY!!!
You will be requires to sign this and return it at your first or second lesson, depending on when it is received. Parental signature will be REQUIRED for any student aged 18 or younger.

* Combination voice/piano lessons are charged the hourly voice rate.
**Coaching sessions are only offered in hourly segments.
